ALL REQUESTS MUST BE PRESENTED AT CALENDAR MEETING BEFORE APPROVAL

Calendar meetings are every Tuesday at 9:30 a.m.  Requests must be presented at the calendar meeting a minimum of TWO WEEKS before the event.  (outside events cannot be considered more than one year in advance).  It is important to note that the categories marked with * are required to complete the form.  Once the form has been successfully completed you will see a message stating that you have successfully completed the information required.
